Contact info for Sales Orders

On the BMS, Sales orders need to have a place for a contact name. Who ordered it? Who placed the order? The person's contact #'s and contact e-mail. This could be changeable like the address is for each sales order. This contact information is crucial for nailing down and identifying *who* placed the order.

Most companies have more than one person who places orders or interacts with the service provider. Presently, phpBMS doesn't support multiple contacts for a single company. This proposed set of text fields could be a step towards that direction. And could be kicked up a notch like you did with the multiple addresses for shipping.

Really a duplicate of #161. We need to implement client multiple contacts, and then figure out how to handle them appropriately on sales orders.
