Abstract "Payment Type" to it's own table

Description
Payment type needs to be abstracted to it's own table, and include things like "type" (e.g. credit card, check/bank) THis way you do not have to have "Discover Card" hardcoded to do the javascript switch hide/show
It also better paves the way for online processing through third parties.
